Project Grants to Address Root Issues

Goal: Address the root causes of one or more social determinants of health among an underserved or marginalized population.  These grants can be used to fill gaps in program services, support collaborative capacity building and advocacy, address health inequities, and reduce health disparities.

  • Grant size: $10,000 – $100,000/year
  • Duration: One-year grants for first-time applicants; prior grant recipients invited to apply for 2-year grants for same project.
  • Additional Eligibility:
    • Priority given to organizations that center people with lived experience in shaping their project goals and activities.
  • Projects focusing on the following issues are especially encouraged:
    • Navigation of services to address social determinants of health.
    • Behavioral health services and prevention (mental, substance use), not otherwise funded through reimbursement.
    • Direct health services (medical, dental, home care) that are not otherwise funded through reimbursement.
    • Improvement of cultural competence/humility among organizational staff, community members and/or volunteers.
    • Language translation and interpretation services that increase access to services addressing the social determinants of health.
    • Professional development for staff and/or volunteers working with underserved and/or marginalized populations.
    • Capital projects (max request of $50,000 per year).

What is the impact that the Partnership wants to support? Grantee has one or more of the following outcomes with project-specific funding:

  • Increased equitable access to health and human services to address social determinants of health.
  • Programs or services reduce health disparities for specific populations.
  • Improved health outcomes.

      • An Audit Report or most recent 990 or 990-N, Current Statement of Financial Position (also called a Balance Sheet), and Current Statement of Activities (also called a Profit-and-Loss Statement)
    • Fiscal Sponsor Information (when the applicant organization is not a 501(c)(3) or a church with Group exemption).
